2 MAKING IDENTITY CARD

3 Step 1 Go to the ‘RT’ and tell if you want to make an identity card and will be given an ‘surat pengantar’ and also ‘surat menikah atau tidak’.

4 Function of RT The RT’s job is to give and keep legal forms for people who wants to make legal documents.

5 Step 2 The ‘RT’ will give you both of the forms and you need to fill in the form.

6 Step 3 You need to go to the ‘RW’ and ask for the signature and legal stamp.

7 Function of RW The RW’s job is to sign and stamp legal documents for residents around the block.

8 Step 4 Bring the form and requirements (such ‘surat pengantar’, photo copy family card, and ‘surat keterangan luar negeri’ if from other foreign country) to ‘kelurahan’ office.

9 Function of ‘Lurah’ ‘Lurah’s job is to collect and keep legal documents related to the government.

10 Step 5 Fill in another form given in the ‘kelurahan’ office which is called F1-07

11 Step 6 Go to ‘camat’ office and have photo session, signing session, eye scan session and also finger printing session.

12 Function of ‘Camat’ ‘Camat’s job is to sign legal documents related to government.

13 Step 8 You will be given a receipt to take the identity card.

14 Step 7 You will be asked to go to back to the ‘kecamatan’ office after 1-2 weeks to take the identity card.

15 Step 9 Take the identity card in the ‘kecamatan’ office and the identity card is done and ready to be used.

16 MAKING FAMILY CARD

17 Step 1 Going to the ‘RT’ asking for the ‘surat pengantar’ for making the family card and also the ‘formulir isian kartu keluarga’.

18 Function of RT The RT’s job is to give and keep new forms for people who wants to make legal forms.

19 Step 2 Fill in both of the form completely.

20 Step 3 Waiting for ‘RW’ to sign the form and give stamp the form.

21 Function of RW The RW’s job is to sign and stamp legal forms (given from the ‘RT’) for the resident around the block who wants to make family card, identity card and etc.)

22 Step 4 Bringing the form and the requirements [Identity Card, ‘Surat Pengantar’, ‘formulir isian keluarga’, photo copy of ‘akta perkawinan, ‘surat kedatangan dari luar negri’’ if from foreign country] needed to ‘Kelurahan’ office.

23 Function of ‘Lurah’ ‘Lurah’s job is to collect data related to legal documents.

24 Step 5 Fill in the form given in the ‘kelurahan’ office and give the form and the requirement needed to the staff in the ‘kelurahan’ office.

25 Step 6 Go to ‘kecamatan’ office and will be given a receipt and will be asken to go to the ‘kecamatan office after 1-2 months.

26 Function of ‘Camat’ ‘Camat’s job is to sign legal documents related to the government.

27 Step 7 Take the family card in the ‘kecamatan’ office and the family card is done and ready to be used.
