2 ILUSTRASI KASUS

3 Anak I, perempuan, 1 tahun 4 bulan, dirujuk dari RS K, dengan keluhan bibir terlihat biru terutama saat menangis sejak empat bulan sebelum masuk rumah sakit, yaitu saat pasien berusia 1 tahun. Selain pada bibir, kebiruan juga terlihat pada wajah. Kebiruan akan menghilang dengan sendirinya setelah sekitar 15 menit. Riwayat kebiruan sebelumnya disangkal. Riwayat sesak napas juga disangkal. Sejak usia 6 bulan, pasien terlihat cepat lelah saat diberi ASI sehingga pemberian ASI terputus-putus karena pasien terlihat belum merasa kenyang. Ibu pasien juga mengeluh berat badan pasien sulit naik meskipun nafsu makan pasien cukup baik.

4 Pada pemeriksaan fisik ditemukan sianosis pada mukosa mulut, bunyi jantung I dan II normal, terdapat murmur ejeksi sistolik derajat 3/6 pada sela iga 2 linea sternalis sinistra, serta jari tabuh pada ekstremitas. Pada tanggal 30 Desember 2013 telah dilakukan echocardiography dengan kesimpulan tetralogy of Fallot sehingga direncanakan kateterisasi jantung, yang telah dilaksanakan pada tanggal 17 Januari 2014, dengan kesimpulan diameter arteri pulmonalis cukup. Dengan demikian, pada pasien direncanakan koreksi total sebagai operasi definitif pada tetralogy of Fallot.

5 Berdasarkan ilustrasi kasus di atas, seorang anak perempuan berusia 1 tahun 4 bulan telah diketahui menderita tetralogy of Fallot dan disarankan untuk menjalani koreksi total sebagai operasi definitifnya. Setelah mengetahui hal tersebut, ibu pasien menanyakan pada usia berapakah operasi tersebut sebaiknya dilakukan supaya anaknya dapat bertahan hidup.

25 - mortalitas pasien tetralogy of Fallot yang dioperasi pada usia di atas 1 tahun lebih tinggi daripada pasien yang dioperasi sebelum usia 1 tahun. - Pada pasien berusia > 1 tahun, peningkatan risiko mortalitas tidak berhubungan dengan pertambahan usia pada saat operasi faktor usia  operasi dapat dilakukan pada usia berapa pun  harapan hidup jangka panjang pascaoperasi yang akan dicapai akan sama perlu dipertimbangkan faktor-faktor lain yang menjadi indikasi operasi pada pasien
